Before our first session, I encourage you to call your insurance and ask the following questions:

1. Is nutrition counseling, also called medical nutrition therapy, covered? (the procedure codes are 97802 and 97803)

2. Which diagnosis codes are covered as preventive; ask specifically about Z71.3. If the visits will not be covered as preventive, is there a deductible, copay, or coinsurance?

3. How many visits are covered? If there is a yearly limit, which month do the visits reset?

4. Is a MD referral or prior authorization required?

5. Are video visits covered the same as in-person visits for the questions above?

6. Is Nathan Slinkard (NPI 1356017230) covered under the plan? If not, what are the out of network nutrition benefits?
